Form 111 is a medical-fitness examination form. Egypt’s Health Insurance Organization lists it first for an employee’s first card, and the Ministry of Health says the health insurance committees do the examination. No official page we could open says where a blank form is issued, so this guide gives who asks for it, the papers that go with it, and who to ask.
Form 111 is a medical-fitness report: a medical examination form stamped «لائق طبياً» (medically fit). Egypt’s Health Insurance Organization (HIO) lists it first among the papers an employee needs for a first health card, and social insurance asks for it when it registers a private-sector worker (both read 23 October 2026).
This guide uses only official pages we could open on 23 October 2026. They say what Form 111 is, who asks for it and who carries out the examination, but not where a blank form is issued, what it costs or how long it stays valid. So this guide gives no fees, and those answers come as questions to ask. Whether you need the form is the Organization’s decision, not ours.
In short:
- Form 111 is a medical-fitness examination form. It is not a prescription.
- HIO lists it first for employees applying for a first card; the National Organization for Social Insurance (NOSI) lists the fitness report on Form 111 among the papers an employer files to register a private-sector worker.
- The Ministry of Health and Population says the examination is done by the health insurance committees. No official page we could open says where a blank form is issued: start with your employer’s personnel office, then the hotline.

What is health insurance Form 111 in Egypt?
Two official pages name it, and a third describes the examination without the number.
- HIO. Its file «تحديث تعليمات استخراج بطاقات التأمين الصحي» (updated instructions for issuing health insurance cards) lists «استمارة 111 كشف طبي مختوم بختم (لائق طبياً)»: Form 111, a medical examination stamped “medically fit” (read 23 October 2026).
- NOSI. Its page on insuring a private-sector worker lists the fitness report on Form 111, issued by the competent medical authority, at the start of the insurance period or a copy of it. A second page, on a worker joining an employer, lists «تقرير اللياقة الطبية للعامل الصادر من الجهة الطبية المختصة (نموذج 111 تأمين صحى)»: the worker’s fitness report, Form 111 (health insurance). On both pages the employer files registration form «س1» within two weeks of the worker starting, with the report attached (read 23 October 2026).
- The Ministry of Health and Population. Its FAQ on getting a health insurance card says «الكشف الطبي يتم بلجان التأمين الصحي» (the medical examination is done by the health insurance committees) and that the result is «لائق طبيا». It does not use the number 111, and the page is undated (read 23 October 2026).
HIO writes «استمارة» and NOSI writes «نموذج», but both mean the form numbered 111. None of the three pages says what the examiner checks or what the form records beyond the stamp, so this guide leaves that out. We take the Ministry’s «الكشف الطبي» to be the same examination because it ends in the same stamp; confirm that with your branch.
Who needs Form 111 for health insurance?
It depends on your category. HIO’s file gives each category its own list of papers. It mentions Form 111 only for employees and contracting workers (read 23 October 2026):
| Category in the file | Form 111 on its list? | What the file says |
|---|---|---|
| Employees: first card (government, public and private sector; Law 148 of 2019) | Yes, the first item | «استمارة 111 كشف طبي مختوم بختم (لائق طبياً)» |
| Employees: renewal | No | An insurance print-out; an employer’s letter saying the contract was renewed (private sector) or that you are still in service (government or public); the establishment’s last payment receipt (business sectors); a copy of your national ID |
| Contracting workers («عمال المقاولات») | A copy, “if any” | A copy of Form 111 stamped fit and approved by social insurance, in which case a full card is issued. With no exam, the card is issued marked «علاجية فقط» (for treatment only) |
| Pensioners and widows | Not listed | Their lists name other papers |
Read 23 October 2026. The file’s text carries no date; its properties show it was made in May 2023. “Not listed” is not the same as “not required”: ask your branch.
Where does Form 111 come from?
The Ministry of Health and Population’s FAQ says the examination is done by the health insurance committees, and that the original and a copy are handed in so the fitness decision can be confirmed (read 23 October 2026; the page is undated). No official page we could open says where a blank Form 111 is issued, which committee you attend, what it costs or how long the stamped form stays valid. NOSI says only “the competent medical authority”. HIO’s home page lists its online services, and none of them is a Form 111 service (read 23 October 2026).
Your employer is part of the process from the start. In HIO’s file the card is issued at the workplace and the employee roster is stamped by the employer; on NOSI’s side the employer, not the worker, files the registration. So ask, in this order:
- Your employer’s personnel office. Ask what they need from you and where they send new staff for the examination.
- HIO’s hotline, 106. Its contact page lists it for urgent inquiries (read 23 October 2026). It is not a medical emergency line. To reach your branch instead, see booking, finding your branch and replacing a lost card.
- The social insurance office your employer registers you with, if you work in the private sector.
Who signs and stamps what for a first employee card?
For an employee’s first card, HIO’s file lists these papers (read 23 October 2026):
| Paper | Who provides or stamps it | Item in the file |
|---|---|---|
| Form 111, a medical examination stamped «لائق طبياً» | The medical authority that examines you. The file does not name it; the Ministry’s FAQ says the health insurance committees | 1 |
| Employee roster form «ت.ص.101», an HIO form, showing the health-insurance share deducted, original plus two copies | Stamped by your employer, approved by NOSI with the Republic’s emblem stamp | 2 |
| Insurance print-out («طابعة تأمينية») showing your law of entitlement | The social insurance office | 3 |
| Copy of your valid national ID, original shown | You | 4 |
| Two recent photographs | You | 5 |
| Proof that the establishment paid its health-insurance share and owes nothing (public and private business sectors) | Not stated; ask your employer | 6 |
| Recent pay details, for guidance | Approved by your employer | 7 |
| Copy of the fixed-term contract (for contracted staff), or the appointment decision with the table of names showing who the card is for | Not stated; ask your employer | 8 |
The file adds that the card is issued at the workplace (item 9), after three consecutive insurance months or six separate ones, the last two consecutive (item 10). Read 23 October 2026; the file dates from May 2023.
In Egypt: HIO, social insurance and Universal Health Insurance
These are different offices. HIO issues the health card and NOSI registers the worker, and Form 111 appears in both lists. Universal Health Insurance is a separate scheme under its own authority; HIO’s file does not cover it. Which one applies depends on your governorate and your category. The map of healthcare in Egypt sets them side by side.
What to ask when you get the form
- Which committee examines me, where is it held, and do I need an appointment?
- Does the form go back to my employer, and does it carry only the stamp or the findings too?
- Is there a fee, and do I get a receipt?
- How long does the stamped form stay valid for my card file?
- If it is not stamped fit, what are my options, and can the examination be repeated?
Ask for a copy of what you sign. How to request copies of your records is in how to get a copy of your medical records.
